What is chronic disease management in primary care?
It is ongoing, relationship-based care for long-term conditions—regular visits, labs and monitoring as needed, medication adjustments, education, and a clear plan so you know what to watch for between appointments.
Which chronic conditions do you help manage?
Common examples include type 2 diabetes, prediabetes, high blood pressure, high cholesterol, asthma and COPD, depression and anxiety, thyroid disorders, chronic kidney disease (early stages), arthritis, and many others. If you are unsure, call and we can help determine whether we are the right fit.
How often will I need to come in?
Frequency depends on how well-controlled your condition is, which medicines you take, and your goals. Some people need visits every few months; others may be stable on a longer interval. We will recommend a schedule that matches your situation.
Do you coordinate with specialists?
Yes. When you need cardiology, endocrinology, pulmonology, mental health specialists, or other consultants, we help with referrals and incorporate their recommendations into your primary care plan.
Is chronic care covered by insurance?
Many chronic care visits are covered like other primary care visits, but coverage depends on your plan, deductible, and network status. We recommend reviewing your benefits or calling the number on your insurance card if you have questions about cost.
What should I bring to my visit?
Bring an updated medication list (or all bottles), your insurance card, recent home readings if you track blood pressure or blood sugar, and any questions you want to make sure we address.
